fix(ui): fix shuffle badge shift, regenerate PNG icons, fix favicon path

- styles.css: remove overflow:hidden from .player-info-wrap — it was
  clipping the player-status-row and shifting the shuffle badge
- index.html: use absolute /icon.svg path for favicon; add PNG fallback
  for browsers without SVG favicon support
- icon-192.png / icon-512.png: regenerated from new equalizer SVG design
